If you move into our practice area and would like to register with us, please pick up our a registration pack at reception.
It is practice policy to ask all patients to provide two forms of identification, one to verify the registration address and one to verify identity.
Forms of identity include: passport, driving licence, visa, work permit, European Health Insurance Card.
Proof of residency can include: recent utility bill, bank statement, recent correspondence from a government body.
For patients from overseas: a passport or ID card.
These lists are not exhaustive; the practice may accept other forms of identification if provided.
Out Of Area Registration
If you live outside of our catchment area but wish to register with us, we may be able to accept your registration as an “Out of Area” patient but without home visiting obligations.
