HOW DO I...
REGISTER WITH THE PRACTICE?

If you move into our practice area and would like to register with us, please complete our a registration form at reception.

It is practice policy to ask all patients to provide two forms of identification, one to verify the registration address and one to verify identity.

Forms of identity include: passport, driving licence, visa, work permit, European Health Insurance Card.

Proof of residency can include: recent utility bill, bank statement, recent correspondence from a government body.

For patients from overseas: a passport or ID card.

These lists are not exhaustive; the practice may accept other forms of identification if provided.

On completion of registration, patients will be asked to make an appointment with one of our practice nurses for a brief new patient medical at which time you can discuss any health problems you may have.
