MY PERSONAL DETAILS?
If
you change your name*, address or contact details, please notify the surgery as soon as possible,
as this will enable us to contact you if the need arises. We find it very
useful to have your telephone number, especially that of your mobile. If you
move out of our area you will be asked to find a new doctor near to where
you have moved.
Please note that if you are having any hospital
treatment you will need to inform the hospital of your change of address
or name.
* We do require evidence of any name change eg a marriage certificate.
