Chronic Disease Management
We offer annual reviews
for patients diagnosed with any of the following conditions:
- Heart Disease
- Diabetes
- Asthma and COPD (Chronic Obstructive Pulmonary Disease)
- Hypertension (high blood pressure)
- Strokes
- Epilepsy
- Mental Health Problems
- Cancer
All nurses receive advance training in chronic disease
management resulting in extensive knowledge and experience in patient
management. If you have been diagnosed with one of the above conditions,
your GP will involve the practice nurse in your care at the earliest
opportunity. Once your condition is stabilised the practice nurse
will assume responsibility for the monitoring and routine follow up
of your chronic disease. If this monitoring reveals any problems that
may require a significant change in therapy, the nurse will refer
you back to your GP. However, if at any time you develop new symptoms
that concern you, please make an appointment to see either the GP
or nurse.
Patients will be sent a letter, at least once a year,
inviting them to attend for review and if necessary to have a blood
test. Please keep this appointment as it is a very important part
of your patient care.
Cervical Screening Services (Smear Tests)
Routine cervical screening is advised for all ladies aged 25-50 every three years and those aged over 50-65 every five years. These are carried out by the practice nurses.
Contraceptive
Services
Patients requiring contraceptive advice
can see the nurse or GP. Any female patients requesting the contraceptive
pill for the first time need to see the GP but subsequently can see
the practice nurse every six months.
Travel Immunisations/Advice
Patients need to complete a travel questionnaire prior to making an appointment for travel vaccinations and advice. These are available from reception. Once completed and returned, reception staff will be able to arrange an appointment with the nurse, please allow three working days' notice.
Please ensure you give yourself enough time to complete a course of vaccinations before travelling, preferably four to six weeks before departure.
Child Health Checks And
Immunisations
Our doctors offer child health checks and the practice
nurses give immunisations. Your child will be sent an appointment to attend. All new babies are
invited for regular check-ups from eight weeks old..
Minor Surgery
Minor operations can be carried out in our treatment
room. Please discuss this with your doctor who will then arrange an appointment for you.
Flu And Pneumonia Vaccinations
Flu clinics usually
run from October onwards each year. All patients aged 65 and over
or deemed to be at risk are invited to attend. Details of the flu
clinics will be advertised in reception from September onwards.
The pneumonia vaccination is also offered to patients in the above category. One injection (good for 10 years' protection) can be given at any time of the year by the practice nurse.
Smoking Cessation
Advice
Patients can see our smoking cessation
counsellor or practice nurses who can offer advice and support to
patients trying to give up smoking. Please arrange an appointment
at reception.
Antenatal Clinics
The community midwives run these
clinics.
Once your pregnancy is confirmed you will need to see the midwife to arrange your referral to a hospital of your choice for antenatal booking.
The midwife will continue to see you regularly throughout your
pregnancy either at the practice, the local hospital or both.
Health Checks
We offer three-yearly health checks
for all our existing patients and annual reviews for over 75 year
olds. These appointments are only available on request and we do not
routinely invite patients for such a check. If you would like to have
a health check please make an appointment with the practice nurse.
